Document SOP and remediation as independent seats

Summary

  • SOP is SopPort during context gather, not a tool under RCA.
  • Remediation is RemediationPort (one HITL-gated action), not a runbook.
  • Master wires them via SOP_AGENT_URL / REMEDIATION_AGENT_URL.

Test plan

  • README seat table matches contracts + master-agent adapter.
